Methodology

How this is compiled,
and where the gaps are.

Each vertical leans on one or two primary published sources. The Concierge Medicine Today Frontier Annual Report surveys over 380 practices. The AAO Economic Survey reports on over 1,200 orthodontists. The ADA Health Policy Institute Survey of Dental Fees reports over 2,100 general dentist respondents with regional bands. The ASPS annual procedural statistics publish average aesthetic pricing nationwide, and RealSelf supplements metro detail for medspa data.

Where a metro sits inside a region with direct sample coverage, the pricing is flagged high or medium confidence. Where a metro is not directly sampled, the national median is adjusted by a cost-of-living factor indexed to 1.00 for the US average. Those readings carry a low confidence flag. The confidence badge is always visible, never hidden under an aggregate score.

p10 and p90 represent the tenth and ninetieth percentiles where published. Where only a range is published, we use the reported low and high as proxies. Cash-pay prices are prioritized over insurance-negotiated rates for specialty medical consults, because cash-pay is the denominator most practice owners care about when setting a fee.

Update cadence

Quarterly. When a source publishes a new annual survey, we integrate it in the next quarterly refresh. A dated “last updated” label accompanies every reading.

Known limitations

Sample sizes vary by vertical and region. Self-reported data skews mid-band. Specialty medical consults have the thinnest public-pricing coverage. Medspa membership pricing varies widely in what is included. COL-bridged metros will drift from ground truth over time.

Concierge medicine

Membership fee medians come from the Concierge Medicine Today Frontier Annual Report, which surveys over 380 concierge practices annually. Initial visit fees are less systematically tracked; we apply regional adjustment to MGMA new-patient consult medians cross-referenced with AAFP DPC survey bands. Panel size data is self-reported by practices and skews toward the mid-band. Metros with a specific sample from CMT (NY, LA, DFW, Miami, DC, SF, Boston, Tampa Bay, Chicago, Houston) carry high-confidence flags; others are COL-adjusted from the national median.

Orthodontics

Adult comprehensive case medians come from the AAO 2022 Economic Survey with over 1,200 respondents. Invisalign case pricing is triangulated from AAO data plus Align Technology provider benchmarks. Two-phase pediatric is less consistently reported; we use the AAO mid-band adjusted for regional fee schedules from ADA HPI. Metros with direct AAO regional sample coverage (Northeast, Pacific, South-Atlantic, Midwest) carry high confidence flags. All other metros are COL-adjusted.

Medspa and aesthetics

Botox per-unit pricing is compiled from RealSelf regional bands cross-referenced with ASPS 2023 procedural statistics. Hyaluronic filler per-syringe pricing reflects Juvederm and Restylane product family medians. Membership pricing is compiled from AmSpa industry reports and is less consistently reported than per-procedure pricing (practices vary in what a membership includes). Coastal metros and major aesthetic markets (NYC, LA, SF, Miami, DC, Boston) are directly sourced; others are COL-adjusted.

General dentistry

ADA HPI publishes fee survey data by region (Northeast, Midwest, South, West) with over 2,100 general dentist respondents. We map each metro to its region and apply the reported regional median with COL adjustment to the metro level. Procedure codes referenced: D2740 porcelain crown, D3330 endodontic therapy molar, D0120 + D1110 exam plus adult prophylaxis, D2962 porcelain veneer. High-confidence flag where the metro sits within a well-sampled regional band.

Specialty medical consult

New-patient consult fees for specialty medicine are less consistently reported than procedural pricing. MGMA Cost Survey publishes 99204/99205 E/M code medians by specialty and region. We map metros to the nearest reported region and apply COL adjustment. Plastic surgery consults often waive the fee on booking; we report the stated consultation fee where charged. All specialty medical data carries medium confidence unless a metro has a direct MGMA regional sample. Cash-pay prices (concierge, plastic surgery) are prioritized over insurance-negotiated rates.
